Coming Home Action Plan

In March 2026, the Scottish Government published the Coming Home Action Plan. This plan outlined the actions needed to resolve cases where people with complex care needs are placed in out-of-area institutional settings, often far from family, community ties, and the opportunity for a fulfilling life. Our CEO Moira Bayne chaired the Housing subgroup as part of the Plan, and we were pleased to see that housing and healthcare working together has been acknowledged as fundamental to the success of bringing individuals home to Scotland.

Coming Home: Housing Options Guide

To support the delivery of the Coming Home Action Plan, the housing subgroup has produced a practical housing options guide for Health and Social Care Partnerships (HSCPs). The guide supports professionals to:

• Explore the full range of housing options, from social housing to private rentals and ownership schemes, ensuring that no option is overlooked
• Provide informed, compassionate support that centres the individual’s preferences, needs, and aspirations, rather than fitting them into pre-existing service models
• Build capacity within Local Authorities by offering guidance, case studies, and tools that promote collaborative working across housing, health, and social care sectors.

Supporting partners across Health and Social Care

As of October 2026, we have received funding from the Scottish Government for a dedicated Coming Home Housing Options Broker. This post will support individual members of the Coming Home cohort by identifying bespoke housing solutions on a one to one basis, as well as providing outreach, engagement and training with local authorities and health and social care partners to embed learning and understanding of Scotland’s complex housing landscape within national and local Coming Home strategies.

We are proud to play our part in supporting Scotland’s Coming Home agenda, providing tailored support to individuals and people involved in their care to explore different housing options.
